Accessing your Health Information

Under Ontario’s Personal Health Information Protection Act (PHIPA), you have the right to access or request a copy of your personal health information (PHI)

How to Make a Request:

  • For Your Own PHI:
You may be asked to provide valid identification to confirm your identity. We assume you are capable of making decisions about your own health information unless otherwise indicated.

  • For Someone Else’s PHI:
We will require the patient’s consent. If the patient is incapable of making decisions, legal documentation (e.g. Power of Attorney for Personal Care) must be provided to confirm your role as the Substitute Decision-Maker.

  • For a Deceased Person’s PHI:
You must provide proof of your legal authority, such as a Certificate of Appointment of Estate Trustee and/or a copy of the Last Will and Testament.
